Ashland Family Practice is excited to announce that we are an approved COVID-19 Moderna vaccine provider through the Oregon Health Authority.  To date we have hosted three drive-through clinic for eligible patients and community members and vaccinated over 1200 people. The remaining clinic is for second doses only. We don't currently have any more first dose clinics scheduled.

Sunday, March 28 for 2nd Doses Only
If you were vaccinated on February 28, please return on Sunday, March 28 at the same time as before to receive your second dose and PLEASE bring the vaccination card you were given AND  download and fill out the Screening and Consent Form (ABOVE) for this clinic as well. This clinic is ONLY for people who received the first dose at our February 28, clinic

We are committed to protecting our patients and community from COVID-19.  As we receive more vaccine we will reach out to the next group of eligible patients. Due to the vaccine stability, we will be unable to give vaccines at regular Office visits.
